About OpNote.net
Operative note templates exist inside every hospital’s Epic system, but they’re institution-locked, inconsistent, and often nonexistent for less common procedures. Residents spend time hunting for templates that should just exist publicly. OpNote.net was built to fix that.
The goal is simple: every surgeon, resident, PA, and NP should have access to a clean, accurate, copy-paste template for any procedure they perform, regardless of which institution they trained at or rotated through.
Who makes this
OpNote.net was created and is maintained by a senior general surgery resident, MD, at an academic medical center. Templates are written from clinical experience and reviewed against ACS clinical guidelines and CMS operative note documentation requirements.
How to use these templates
Every template page has a Copy Full Note button that copies clean plain text ready to paste into any EHR. An Epic SmartPhrase version is also available. Import it once into your Epic SmartPhrase Manager and use it for every case.
